SA Health has admitted it failed to appropriately follow review recommendations which found serious deficiencies at its state-run Oakden nursing home in 2008. The health facility in Adelaide’s north-east is now set to close, after an independent investigation by South Australia’s chief psychiatrist found cases of mistreatment, over-medication and inappropriate restraint of residents. The earlier report was done by Victorian consultants Dr Simon Stafrace and Alan Lilly after the Commonwealth Department for Health and Ageing sanctioned the nursing home in late 2007.
